Abstract
Nyeri persalinan merupakan pengalaman yang umum namun menimbulkan ketidaknyamanan signifikan bagi ibu bersalin, khususnya pada kala I. Berdasarkan pengamatan di lapangan, sebagian besar pasien memilih tindakan operasi sesar karena tidak mampu menahan nyeri persalinan. Dari sekitar 10 pasien, sebanyak 5 hingga 6 pasien meminta dilakukan operasi sesar, dan mayoritas di antaranya adalah ibu primigravida. Fenomena ini menunjukkan bahwa ketidakmampuan dalam mengelola nyeri persalinan, khususnya pada ibu yang baru pertama kali melahirkan, menjadi salah satu faktor pendorong meningkatnya angka persalinan dengan operasi sesar. Selain itu, jika nyeri persalinan tidak tertangani dengan baik, hal ini dapat menimbulkan stres fisik dan emosional pada ibu yang mengganggu kemampuan mengejan, pernapasan, dan kemajuan persalinan, meningkatkan respon simpatis serta risiko trauma psikologis, menurunkan suplai oksigen ke janin yang berpotensi menyebabkan distress janin, asfiksia, dan komplikasi lainnya, serta memperpanjang proses persalinan dengan kontraksi tidak efektif, sekaligus meningkatkan risiko intervensi medis dan perdarahan postpartum.Penelitian ini bertujuan untuk mengetahui perbedaan intensitas nyeri pada ibu dalam persalinan kala I sebelum dan sesudah dilakukan teknik Endorphine Massage di UPTD RSD dr. H. Soemarno Sosroatmodjo, Tanjung Selor, Kabupaten Bulungan, Kalimantan Utara. Penelitian ini menggunakan metode kuantitatif dengan desain pre- experimental pretest and posttest one group design. Jumlah sampel sebanyak 20 responden yang diambil menggunakan teknik purposive sampling. Instrumen penelitian meliputi SOP Endorphine Massage dan lembar observasi skala nyeri Numeric Rating Scale (NRS). Hasil penelitian menunjukkan bahwa terjadi penurunan intensitas nyeri hal ini ditunjukkan dengan penurunan angka kejadian nyeri yaitu pada nyeri sedang sebelumnya 70% responden, dan setelah dilakukan intervensi, sebanyak 60% responden. Berdasarkan hasil uji Paired Samples Test diperoleh nilai t sebesar 17,085 dengan p-value 0,000 (p < 0,05), yang menunjukkan terdapat perbedaan yang signifikan antara intensitas nyeri sebelum dan sesudah intervensi, dengan selisih rata-rata sebesar 2,18888. Temuan ini memperkuat teori bahwa teknik Endorphine Massage mampu merangsang pelepasan endorfin alami tubuh yang berfungsi sebagai analgesik alami. Dengan demikian, intervensi ini dapat diterapkan sebagai alternatif efektif dalam manajemen nyeri persalinan. Labor pain is a common yet significantly discomforting experience for women, especially during the first stage of labor. Based on field observations, the majority of patients choose cesarean section due to their inability to endure labor pain. Out of approximately 10 patients, around 5 to 6 request a cesarean delivery, with the majority being primigravida mothers. This phenomenon indicates that the inability to manage labor pain, particularly among women giving birth for the first time, is one of the contributing factors to the increasing rate of cesarean deliveries. Furthermore, if labor pain is not properly managed, it can cause physical and emotional stress in the mother that interferes with her ability to push, breathe, and progress during labor, increases sympathetic responses and the risk of psychological trauma, reduces oxygen supply to the fetus which may lead to fetal distress, asphyxia, and other complications, as well as prolongs labor due to ineffective contractions, while also raising the risk of medical interventions and postpartum hemorrhage. This study aimed to determine the difference in pain intensity among mothers in the first stage of labor before and after the application of the Endorphine Massage technique at UPTD RSD dr. H. Soemarno Sosroatmodjo, Tanjung Selor, North Kalimantan. A quantitative research design was used, employing a pre-experimental method with a one-group pretest-posttest design. The sample consisted of 20 respondents selected through purposive sampling. The research instruments included the Standard Operating Procedure (SOP) for Endorphine Massage and a pain measurement sheet using the Numeric Rating Scale (NRS). The research results show a decrease in pain intensity, as indicated by a reduction in the incidence of pain from 70% of respondents experiencing moderate pain before the intervention to 60% of respondents after the intervention. The Paired Samples Test revealed a t-value of 17.085 with a significance level of 0.000 (p < 0.05), indicating a statistically significant difference in pain intensity before and after the Endorphine Massage. The mean difference of 2.18888 also demonstrated a substantial reduction in pain intensity following the intervention. These findings support existing theories that Endorphine Massage can stimulate the body's natural endorphin release, which acts as a natural analgesic. Therefore, this technique can be considered an effective alternative for labor pain management.
